Patient has confirmed MRSA bacteremia and the physician has ordered vancomycin 1g iv q12h. MD orders for Pharmacist to follow the vancomycin dosing.

Pt is a 55-year-old male, 65kg, SCr 55 mcmol/L, height 6’. Is the dose of 1g appropriate and is the interval reasonable? When will you order a trough and what target are you aiming for?

How would you manage/ prioritize this situation?
