Chemo-IO means:

Chemotherapy + Immunotherapy given together

In oncology practice, Chemo-IO specifically refers to cytotoxic chemotherapy combined with immuno-oncology (IO) agents, almost always immune checkpoint inhibitors.

What Each Part Does

Chemotherapy

  • Directly kills rapidly dividing tumor cells
  • Causes tumor antigen release
  • Can reduce immunosuppressive cells in the tumor microenvironment

Immunotherapy (IO)

  • Reactivates T-cells to recognize cancer
  • Sustains long-term immune control
  • Most commonly PD-1 / PD-L1 inhibitors

Together, chemo can make tumors more visible to the immune system, enhancing IO efficacy.

Common Chemo-IO Examples (High-Yield)

Non–Small Cell Lung Cancer (NSCLC)

Small Cell Lung Cancer (SCLC)

Triple-Negative Breast Cancer

Head & Neck SCC

Biomarkers Often Used with Chemo-IO

  • PD-L1 expression (not always required)
  • Tumor mutational burden (TMB)
  • MSI-H / dMMR (GI cancers)

Important: Chemo-IO can be used even if PD-L1 is low or negative, unlike IO alone.

Toxicity Considerations (Very Important for Pharmacy)

You get BOTH:

Chemo Toxicities IO Toxicities
Myelosuppression Immune-related AEs
Nausea/vomiting Colitis
Alopecia Pneumonitis
Neuropathy Endocrinopathies

Key rule:

  • Chemo toxicity → dose adjust
  • IO toxicity → hold drug + corticosteroids
