Doxepin Oral Rinse (Swish & Spit)
Class: Tricyclic antidepressant (TCA) – topical use in oral cavity.
Mechanism of Action (Topical)
- Blocks histamine H1 and H2 receptors and adrenergic/nociceptive pathways, providing local analgesic and antipruritic effects.
- Reduces pain from oral mucositis or ulceration in cancer patients undergoing chemotherapy or radiation therapy.
Indications in Oncology
- Painful oral mucositis from:
- Head and neck radiation therapy
- Chemotherapy-induced mucositis
- Off-label use for oral pain management in palliative care when systemic analgesics are insufficient or to avoid systemic side effects.
Formulation / Administration
- Oral rinse (swish and spit), compounded in concentrations such as 5 mg/mL (varies by institution).
- Typical dose: 5–10 mL, swish in mouth for 30–60 seconds, then spit out, up to 4 times daily.
- Do not swallow (minimal systemic absorption).
Adverse Effects (Topical)
- Local: bitter taste, drowsiness if swallowed, transient oral numbness
